Strategic Implementation Framework for Sustainable E-Health Deployment in Fragile Healthcare Systems: Evidence from the Democratic Republic of Congo

The successful adoption of e-health systems does not automatically translate into effective implementation, particularly in fragile healthcare environments characterized by limited infrastructure, institutional constraints, and resource shortages. This study proposes a comprehensive implementation strategy for sustainable deployment of e-health systems in the healthcare sector of the Democratic Republic of Congo.
Building on an empirically validated adoption model and a context-aware information systems architecture, the study develops a strategic framework that integrates technological readiness, organizational capacity, policy alignment, and stakeholder engagement. The framework emphasizes phased deployment, capacity building, governance mechanisms, and continuous evaluation to ensure long-term sustainability.
The proposed strategy addresses key barriers to implementation, including inadequate ICT infrastructure, limited digital skills among healthcare professionals, financial constraints, and fragmented health information systems. It also highlights the importance of leadership commitment, regulatory support, and inter-institutional coordination.
By bridging the gap between technology adoption research and real-world implementation practices, this study contributes to the digital health literature and provides practical guidance for policymakers, healthcare institutions, and system designers. The findings are particularly relevant for developing countries seeking to modernize healthcare systems through sustainable digital transformation initiatives.
